Canada Maple Leaf Card Replacement Process

Obtain the form and fill it out on the official website of the Immigration Bureau, or apply for a supplementary form by letter or phone (only forms that are not available online, and original copies must be requested), usually received within 1 to 2 weeks.

3) Copies of passport, landing paper, driver’s license, etc.

For minors, birth certificate documents must be submitted. If there is a guardian, guardianship-related documents must be presented.

If the spouse is an employee of the Canadian branch working overseas, the spouse’s employment certificate and identity document must be submitted.
