Fill in the content and post photos realistically; 3. Go to the bank to pay for changing your card; you can get a new maple leaf card from February to April.

3. Documents required to apply for replacement card: 1. Complete application form ([], [INM 5445] and payment form 5401, etc.); 2. Copy of existing maple leaf card; 3. Copy of passport, login paper, driver's license, etc.; 4. If you change your name, the official name change file will be published; 5. If you are under 18 years of age, a birth certificate is required; if you have a guardian, guardianship documents will be provided; 6. 2. The methods for applying for a permanent resident card are: Download - Print - Fill in - Register; b. Download - Fill in parts on your computer - Print - Fill in forms - Register; c. Call CIC to make a reservation, they will mail a complete set of documents and fill in the signature upon receipt; d.
